Davisville Medical Clinic
Effective January 1, 2026
This policy explains how Davisville Medical Clinic handles personal health information, and what this website does and doesn't collect.
Who we are
Davisville Medical Clinic is a medical clinic at 1901 Yonge Street, Unit 4, Toronto, ON M4S 1Y6, and a division of Davisville Medical Centre. Our physicians are health information custodians under Ontario's Personal Health Information Protection Act, 2004 (PHIPA). We follow PHIPA when we collect, use and share your personal health information.
Information we collect at the clinic
When you receive care from us, we collect the information we need to treat you and to bill OHIP. This includes:
- your name, date of birth, address, phone number and Ontario health card number
- your health history, symptoms, test results, prescriptions and the notes your physician writes
- information from other health care providers involved in your care, such as labs, specialists and hospitals
How we use and share it
We use your information to provide and coordinate your care, to bill the Ontario Health Insurance Plan (OHIP), to run the clinic and improve our care, and to meet our legal and professional obligations.
We share your information with other health care providers in your circle of care, such as a laboratory, your pharmacy, or a specialist we refer you to, unless you tell us not to. We share it with anyone else only with your consent, or where the law requires or permits it, for example reporting certain diseases to public health.
We do not sell your information, and we do not use it for marketing.
How we protect it
Your record is kept in a secure electronic medical record system. Access is limited to staff who need it for your care or for clinic operations, and everyone who works here is bound by confidentiality. We keep records for as long as the College of Physicians and Surgeons of Ontario requires, then destroy them securely.
If your information is lost, stolen or accessed without authorization, we will tell you as soon as reasonably possible and report it to the Information and Privacy Commissioner of Ontario where PHIPA requires.
Your rights
- See your record. You can ask for access to, or a copy of, your health record. We may charge a reasonable fee for copies.
- Correct your record. If you think something in your record is wrong, you can ask us to correct it.
- Limit sharing. You can ask us not to share some or all of your information with other providers. We'll explain how that may affect your care.
- Withdraw consent for uses that rely on your consent, at any time.
To make a request, contact our Privacy Officer (below), call 416-483-0102 or ask at the front desk.
